Job Summary

The Community Resource Center (CRC) Community Representative II reports to the Center Administrator and is responsible for participating in outreach activities aimed at increasing the Community Resource Center's visibility in the community and increasing participation in the CRC programming. This role will be 50% field-based and will include traveling to various organizations and providing outreach which may include presentations on health coverage program eligibility, promotion of the Center and related services, participation in health fairs and health education events. This position is responsible for establishing and strengthening linkages with surrounding community based organizations, physicians, schools, businesses, faith based organizations, public agencies and other community partners. Outreach representative will work closely with Administrator to identify partners and establish partnership agreements. Thia position is responsible for identifying external groups and multi-ethnic agencies in the community. Establish and maintain partnerships with community agencies to ensure a mutually beneficial working relationship. This position will also work in close collaboration with internal L.A. Care Health Plan departments. Extensive local travel will be required on a daily basis.

Duties
Participates in community events such as Health Fairs, Open Houses, etc. Conduct presentations to promote Community Resource Center and the services provided to assist in engaging community and member participation. (35%)

Establishes and strengths linkages with surrounding community based organizations, physicians, schools, businesses, faith based organizations, public agencies, other community partners and internal departments. (35%)

Serve as a liaison to members, providers and community partners. (10%)

Completes all assigned reports or documentation to monitor and track performance. (10%)

Perform other duties as assigned. (10%)

Education Required
High School Diploma/or High School Equivalency Certificate
Education Preferred
Associate's Degree
Experience
Required:
At least 0-1 years of previous community outreach experience.



Experience working with an ethnically diverse population.

Skills

Required:
Knowledge of application assistance for enrollment in any of L.A. Care's product offerings.



Strong verbal and writing communication skills.



Must have strong time management and organizational skills.



Excellent PC Skills (MS Office) with emphasis on data entry, spreadsheet, and word-processing.


Strong persuasion skills.



Preferred:
Bilingual in one of L.A. Care Health Plan’s threshold languages is highly desirable. English, Spanish, Chinese, Armenian, Arabic, Farsi, Khmer, Korean, Russian, Tagalog, Vietnamese.
